I will provide an invoice at the end of each month with information required by Insurance Plans that you can submit for reimbursement. This invoice will include the necessary procedure codes, diagnoses and dates of service information. This can be submitted online with a claim form provided by your insurance to obtain any eligible coverage offered by your plan.
